Your personal care team includes:
Your Physician
Reviews the Plan of Care, including medications and treatments. Certifies homebound status and makes appropriate adjustments, and remains involved in your treatment.
The Director of Patient Care Services (RN)
Supervises and directs the team in developing a plan of care that fits your needs and collaborates with your physician.
Your Nurses (RN and LVN)
Evaluate and determine your medical need and render treatment, instruction, and teaching in accordance with the Plan of Care, which your attending physician approves.
A Certified Home Health Aide
Assists patients with activities of daily living, such as grooming, bathing, hygiene, eating, and light housekeeping.
Physical Therapist
Reviews the patient’s physical condition and creates a physical therapy program to help you with pain management, safe ambulation, range of motion, transfer, gait, and durable medical equipment usage.
Occupational Therapist
Work in conjunction with other disciplines to help the patient’s adaptive functioning in activities of daily living.
Speech Therapist
Addresses problems in speech and improves communication ability.
A Medical Social Worker
Provides counseling, resources, and support for the patient and their family’s emotional, social, and financial needs.
Registered Dietitian
A food and nutrition expert who consults with you and helps manage your nutritional and food therapy. Makes recommendations on a diet. Most registered dietitians work in the treatment and prevention of disease by administering medical nutrition therapy as part of medical teams.
